Templates and forms
- Student Health Support Plan — template for schools planning medical or health-related support at school, including continence care
- Functional Needs – Medical Advice Form — may be used by the treating health team to describe a student’s functional and personal care needs, such as toileting, personal hygiene and menstrual health. This can be used to inform the Student Health Support Plan
- Toileting Care and Personal Hygiene Learning Plan — schools may choose to use this plan as a suggestion, to maximise opportunities for students to self-manage components of their personal care support, as far as possible, in relation to toileting, and to acknowledge the learning that has occurred when success is achieved
